How We Conduct Care Assessments

At Rainbow Direct Care we believe in transparency and straight forward talking. From the initial enquiry, a member of our Care Assessment Team will visit you and carry out a full assessment of your care needs. If you are not comfortable with this being carried out at your home the assessment can be carried out over the telephone, online assessment form or we can arrange for you to pop into our offices.

This will give us an opportunity to get to know you and understand any personal needs and preferences as well as the support you will require.

Once the initial assessment has been agreed our Assessments Team will liaise with our Care Managers and Team Managers to propose Care Arrangements and Care Packages.

This can be reviewed with you, your family and other professional or agencies to ensure we have covered everything.

Continuity of Care

Once the Care Plan has been agreed, our Care Team will arrange to meet with you, your family and your loved ones. We will explain the Care Service Delivery and introduce your carer.

In most cases you will always have the same carer visiting you. We do this to avoid confusion with the individuals and avoid disrupting any routines.

There may be instances when it will not be possible for your allocated carer to attend maybe due to sickness or family holidays. But we will try as much as possible to ensure you will get the same carer all the time to enforce continuity of care and building strong relationships between client and carer.

Ongoing Care Assessments

All our Care Plans are regularly reviewed and constantly monitored to ensure that they are able to meet your care needs.

Where there is need you and family will be consulted with proposals to change or amend care packages in response to changing needs.

We also welcome your views and feedback, and where you feel your needs have changed we are happy to discuss how we can adapt our services to that change.

Services, Care Plans and Care Packages will also change in response to changes in legislation and government policy. In this case you and your family will be consulted on the proposed changes and any impacts discussed.
